ive been using accutane and its still not enough for my skin

i know chatgpt says not to, and the general consensus is one or the other, but can i possibly use tretinoin alongside my isotretinoin? ive been using accutane for a few years now and it hasnt been enough. i have rlly oily skin.

i think the dose might not be the problem but rather the course time, i think low dose iso for some years might fix it forever, without really compromising liver function tbh.
Based on the data I have read, the dose can actually make a huge difference despite time, people believe they can do a 20mg a day for a 3 years and get the same results as someone doing 60mg for year (mind both have the same accumulative dose) but when played out, the 60mg dose will always win in terms of future relapse rate and gland size reduction overall. A higher concentration dose generally leads more suppression as 20mg can only suppress so hard even tho its a longer treatment, while 60 can suppress even harder thus leading to less relapse in acne...

yes, just be careful with the irritation, isotretinoin sometimes takes some time to kick in, i did exactly the same as what you are considering doing and got very good results, just make sure to leave enough time for you skin to adapt, if i was you i would use it every 2x days, either that or up the dosage of isotretinoin

How many mgs accutane?

ive done accutane for the last 2 years or so, ranging from 40-60 mg mostly. maybe done a few months at 20mg but not much. my skin is really oily and my acne i believe is hormonally genetic

i would just do 10-20 mg permanently, it does not look like it's going to stop, just do 10-mg permanently and pair it with dutasteride to see if that helps keep the oil glands controlled, i would also prioritize use a cleanser that is designed for super oily skin.

i would incorporate dutasteride 100% as DHT dysregulates the oil glands production, so you would be nuking 100% the oil glands production, as it seems isotretinoin wont be enough. While simultaneously taking care of your hair follicles. You would be targeting the issue from 2 different sides, i would give that a try.

ive gotten bloodwork, my dht is on the lower end

i got desperate one time and blasted dut for 3 months and saw no change :/

maybe ill do it again

yeah my thyroid is really fucking low should I take thyroid meds?

didnt know thyroid had a molecule to do with acne
not directly, but can highly influence hormone balance, i would consider taking thyroid medication and going for a consult with a doctor, meanwhile just keep doing 10-20 mg of isotret plus tazarotene or tretinoin, i would still incorporate a 5AR blocker so dutasteride.

You’ve been using accutane for years? What dose, 0.5mg a day? Just increase the dosage to 40-60mg

first of all i need to tell you i'm not a doctor, second i have great news and bad news to tell you, the great news is that all your bloodwork is mostly unremarkable, nothing really stands out, no signs of diabetes, kidney/liver dysfunction, anemia, infection, or major hormonal imbalances, all biomarkers are within normal ranges except for the thyroid function, here's where the bad news come in, as for what it seems your a have subclinical hypothyroidism, your TSH is fairly elevated at 6.58, now the thyroid function affects a lot of stuff, hormone balances, skin cell turnover and the epidermal barrier function amongst a lot of other things and also how the facial oil glands behave, as this bloodwork doesn't have an antibodies test it's not possible to tell the cause behind your hypothyroidism, it could be either an autoimmune disease, iodine deficiency, medications etc, the thyroid is one of the most under looked causes of severe cystic and chronic acne and has a direct correlation and causation with acne, now this is as far as i can help you cause im not a doctor, but i would 100% suggest you go with an endocrinologist cause subclinical hypothyroidism can become worse over time and affect your brain chemistry and libido cause it affects the SHBG. Mark as solution.
