PDE3 inhibitors - More than a Heart failure medication?
Hello everyone,
Introduction
At first glance PDE3's seem like a regular vasodilator but after researching, they are one of the most pharmacology interesting vasodilators i've seen.
There is a whole lot to them and I'm suprised how little they are talked about.
Main Uses/Mechanisms and Types
A PDE3 inhibitors are medication that stop the action of phosphodiesterase enzyme PDE3.
they are mainly used for heart failure therapy and cardiogenic shock.
it produces a dilation of blood vessels and increase the strength of the hearts muscle contractions and increase heart rate
Basic mechanism :
PDE3 breaks down cyclic AMP . Inhibiting it -> higher cAMP -> smooth muscle relaxation + increased cardiac contractions
Types: these are the ones approved for human medical use
cilostazol , milrinone enoximone, pimobendan (only in Japan tho)

Other benefits and uses:
Anti-depressant
i came across a study that used PDE3 ihibitors in mice with post stroke / chronic mild stress depression in mice after a stroke.
what they found was very interesting.
After dosing the mice with PDE3 inhibitors the result was "reduction of all depressive behaviours examined"
they concluded that PDE3 may have anti-depressant effects on post-stroke depression through inhibition of neurodegeneration
in the primary lesion and secondary extrafocal sites and promotion of neurogenesis.
These beneficial effects on post-stroke depression may be involved in activation of CREB/BDNF signaling.
