ISOTRETINOIN EXPLAINED

I've seen numerous videos online of people fearmongering isotretinoin (accutane) and would like to debunk these claims.

Isotretinoin (accutane) is a systemic retinoid used primarily for acne and is widely regarded as the most effective therapy for acne. Accutane works by reducing sebum production in the skin by shrinking sebaceous glands.





Common Fears

(pussy fears not included)
- Stunts height growth 😂
- Joint pain and Musculoskeletal issues
- Liver Damage
- Purging


Isotretinoin is a vitamin A–derived retinoid, and very high retinoid exposure has been associated with premature growth plate closure in some cases, however don't be fooled.
Most reported cases of growth plate issues occurred in:

- very high doses
- prolonged multi-year therapy
- young children
- neuroblastoma treatment

Isotretinoin can influence musculoskeletal tissues because retinoids affect collagen turnover and bone metabolism. Some cases report:

- healthy lifestyle
- fish oil
- magnesium glycinate
- stretching

Isotretinoin is metabolized in the liver, which leads some to assume it significantly strains the liver.

How to minimize liver stress on isotretinoin
- avoid alcohol
- monitor AST, ALT, and triglycerides
- maintain basic metabolic health


When isotretinoin begins to normalize follicular turnover and reduce sebum production, some may experience a temporary worsening of acne(purging).

How to avoid purging
- Start at a low dose: ex 20mg ed
- Take an antihistamine
- Gentle skincare: Cleanser, Moisturizer, Sunscreen






Personal Experience
I've been on Accutane 20mg ed for about 3 months.

Within the first 3 weeks I noticed significantly drier skin and less whiteheads, along with a slight fade in my PIE.
By 6 weeks (month and a half) I noticed almost no active acne and no closed comedones.
By 8 weeks my skin plateaued
By 12 weeks I have no active acne and my PIE is starting to fade completely.







Dosing

For most, I would recommend starting with 20mg ed and reassessing yourself about 1-2 months in. If the effects seem to be minimal bump up to 40mg ed.
Reminder that accutane is a weight based medication and ones dose may not be the same as anothers.

Dosing Guide
Daily dose (mg) = body weight (kg) × target mg per kg per day
Typical target mg per kg per day: 0.4-0.8 mg per kg per day
