PDE3 inhibitors - More than a Heart failure medication?



Hello everyone, fent fent in one of my threads said to look into PDE3 inhibitors so i did a dive on them





Introduction



At first glance PDE3's seem like a regular vasodilator but after researching, they are one of the most pharmacology interesting vasodilators i've seen.
There is a whole lot to them and I'm suprised how little they are talked about.







Main Uses/Mechanisms and Types​




A PDE3 inhibitors are medication that stop the action of phosphodiesterase enzyme PDE3.
they are mainly used for heart failure therapy and cardiogenic shock.
it produces a dilation of blood vessels and increase the strength of the hearts muscle contractions and increase heart rate​

Basic mechanism :​


PDE3 breaks down cyclic AMP . Inhibiting it -> higher cAMP -> smooth muscle relaxation + increased cardiac contractions

Types: these are the ones approved for human medical use​

cilostazol , milrinone enoximone, pimobendan (only in Japan tho)
this is the boring surface level md shit, onto the good stuff now​

Other benefits and uses:​

Anti-depressant​



i came across a study that used PDE3 ihibitors in mice with post stroke / chronic mild stress depression in mice after a stroke.
what they found was very interesting.
After dosing the mice with PDE3 inhibitors the result was "reduction of all depressive behaviours examined"
they concluded that PDE3 may have anti-depressant effects on post-stroke depression through inhibition of neurodegeneration
in the primary lesion and secondary extrafocal sites and promotion of neurogenesis.
These beneficial effects on post-stroke depression may be involved in activation of CREB/BDNF signaling.

respiratory disease​

a pubmed study from 2009 found that the muscle relaxment effect of PDE3'smay help
diseases such as asthma

Cancer:​

yeah ngl the evidence for this is weak but i might as well add it in.
some studies may suggest that cAMP singnaling influences tumor growth indirectly
PDE ihibition may alter cancer cell signally some how. link is very weak tho
