- Medial Fat Pad (The soft tissue pad closest to the medial canthus)
- Central Fat Pad (The soft tissue pad that sits directly below the middle of the lower lid)
- Lateral Fat Pad (The soft tissud pad closest to the lateral canthus of the eye)
What the 3 fat pads consist of

- The three fat pads consist of capillaries, lymphatic drainage channels, clusters of white adipocytes (fat cells), vessels, and septa (connective tissue walls made of collagen fibers)
- The medial fat pad consists of more fibrosis which makes it firmer and also consists of less carotenoid than the central/lateral fat pads which makes the medial fat pad whiter/paler in color
- It’s harder for the medial fat pad to protrude forward and contribute to eye bags since it’s firmer + lighter in terms of volume + it’s confined by the inferior oblique muscle so it can’t really “go anywhere” in a way, even when the orbital septum weakens
- The central fat pad is the easiest fat pad among the three to protrude past the orbital septum and contribute to eye bags. Going off what we know about the medial fat pad, the central fat pad is the easiest to protrude due it higher in volume + less firm because of lack of septal support + lack of protection from a barrier such as the fascis sheath of the inferior oblique muscle or the arcuate expansion
How the 3 types are seperated -
- To begin with, the lateral and central fat pads are separated by the arcuate expansion which is a connective tissue that forms a curve, separating the central and lateral fat pads
- The arcuate expansion serves as a collagen-rich barrier that originates from the orbital septum
- The arcuate expansion starts in the same region as the inferior oblique muscle and forms somewhat of an “S’ horizontally
- The central and medial fat pads are separated by the fascial sheath of the inferior oblique muscle which forms somewhat of a ribbon shape
- Think of each fat pad as its own little separated compartment that make up the under eyes
