so certain roids have anticatabolic effects through altered cortisol signaling indirectly via gene expression
however glucucorticoid isnt the only receptor cortisol goes in it also goes into a receptor called the mineralocorticoid receptor which is highly expressed in kidneys and the brain and in other tissues
now there is an enzyme called 11β-HSD2 which converts cortisol into a weaker, cortisone in the kdineys, this is a protection mechanism that is in place to prevent the cortisol from reaching the receptor and breaking the electrolye ratio in ur body since mr has MUCH HIGHER receptor affinity to cortisol than regular gr
however what this means is that there is gonna be a high local concentration of cortisol in places where there is no 11β-HSD2 aka the brain which is pretty damaging increasing oxidative stress markers etc,
what can we do? well we can take direct MR blockers like Eplerenone to prevent high local concentrations of mr in the brain and other tissues without 11β-HSD2
btw this is ESSENTIAL if ur taking a glucucorticoid receptor antagonist like mifepristone (i have talked abt this in another thread) since there will be so little cortisol binding to the gr however there is still cortisol in the blood even getting upregulated by the adrenal glands due to receptor blockade which is gonna get accumulated in the mr in high concentrations.
