DAO Botox is often used to correct the appearance of downturned commissures and also helps with a perceived wider intercommissural breadth in most people.
Botox works by preventing the neurotransmitter acetylcholine from being released into the synaptic cleft and without acetylcholine signalling the muscle to contract , the targeted muscle weakens or relaxes.
The effect is localised and lasts about 3 - 4 months usually.
This can be sort of thought of as a less invasive , less powerful version of a commissuroplasty and can easily be performed DIY
Even if your commissures are neutral this can be beneficial for you
Several direct studies and peer-reviewed papers link downturned oral commissures to perceptions of increased age, negative emotions and reduced attractiveness whilst upturned mouth corners are associated with youthfulness and positive traits such as friendliness , approachability and higher attractiveness
Additionally people with upturned oral commissures were also perceived as more intelligent
Generally 2–6 units of botox is used per side
I'd recommend going doing about 3 units on each side , it is better to go conservative , you can always fill up more but cannot undo what you've done.
The 2 most common sites for DAO botox are the following
It is generally better to use one injection site and use the ones on the top and a general rule of thumb according to practitioners seems to be about 1 cm lateral and inferior to the oral commissure although you should directly assess the muscle on your own anatomy by frowning and seeing where the muscle lies.
It is better to use the injection sites on the top because using the lower injection sites have much higher risk of hitting strands of the platysma muscle and the depressor labii which can cause an asymmetrical smile.
Generally an insulin syringe is used (30-31G) is fine
You typically want to go about 2–5 mm deep when you inject
Avoid going any deeper than 5-6mm to prevent diffusion to adjacent lip depressors
Optionally for numbing you can use and leave on for about 30 - 60 minutes before begin treatment
