Dutasteride
Dutasteride inhibits type I and II 5-α-reductase, achieving deeper DHT suppression than finasteride. Trials report greater increases in hair counts and cosmetic improvement, particularly in advanced thinning. It is a high-potency pharmacologic option used when maximal DHT control is prioritized.

Dual isoenzyme inhibition (types I/II) serum DHT suppression often (90%). Type I 5-AR expression in scalp/sebaceous glands adds theoretical advantage for vertex and frontal preservation.
Head-to-head and dose-ranging studies demonstrate superior efficacy vs. finasteride on hair counts and investigator assessments. Benefits accrue through 6–12 months with continued gains thereafter.
Often used after inadequate response to finasteride. Stack with minoxidil; keep ketoconazole as adjunct for scalp environment. Consider medical supervision given depth of systemic DHT reduction.
