The main "deadly" effect from androgens is going to be left ventricular hypertrophy(growing of the heart)
The 2 main drugs we can regress or give a decent amount of stopping LVH(Left ventricular hypotrophy) are going to be telmisartan and Eplerenone
Telmisartan is an Angiotensin II type 1 receptor blocker. (Angiotensin ll is responsible for raising blood pressure and constricting blood vessels and get up regulated by androgens
It is the longest-acting ARB meaning constant blockage unlike other ARBs like losartan. with high affinity for the AT1 receptor giving it strong RAAS down regulation which basically is helping things like aldosterone, water retention and oxidative stress stay regulated.
The second drug to use in stopping LVH is going to be eplerenone. Eplerenone is an aldosterone antagonist. In basic terms it slows down aldosterone's effects in the kidney, heart, and blood vessels.
Referencing this study we can see that even a low dose of 50mg daily of eplerenone cut LV mass from 155g to 136g. Which was significantly better than any blood pressure reducing drug. Why eplerenone is such a good drug for body builders is when you take androgens, aldosterone increases and tells heart cells and fibroblasts to grow and retain sodium and water via the kidneys. So by blocking these receptors you reduce hypertrophy of LV cells and make the walls of your heart more compliant.
The only problem you could run into running both these drugs is they both increase potassium but this easily avoidable by taking hydrochlorothiazide
